Membership

HFW Longevity Membership: a year-long, measurement-driven approach to health optimization.

For patients who want more than a one-time test, consultation, or treatment, the HFW Longevity Membership creates an ongoing system for understanding what is changing in your health, putting a plan into action, measuring progress, and adjusting that plan over time.

Membership is optional. You do not need to become a member to work with HFW or to be considered for an individual service or clinically appropriate therapy.

What patients say

All Reviews
★★★★★

“I have been working with Dr Landi for several months now. He is extremely knowledgeable, attentive and responsive. He actually listens which is rare in medicine these days. I’m excited to see what the results are from my protocol in a couple of months. Highly recommend his practice if you’re looking into functional medicine.”

Fatimah Fahmy
★★★★★

“Dr. Landi is highly knowledgeable and skilled in the field of functional medicine, but he also possesses a genuine empathy and compassion that is so often lacking in the traditional healthcare system. Dr. Landi connects to each of his patients on a personal level, works with you every step of the way, and makes you feel supported while navigating challenging health obstacles.”

Lauren Griffith
★★★★★

“I found out about the practice of Functional Dr’s last year and realized this is what I need! Finding Dr Landi so close to home was awesome. We appreciated his candor, knowledge and engaging personality. He exuded caring and interest in me and my situation. I feel he had repeatedly gone the extra mile for me so our collaboration is successful.”

Why Patients Choose Membership

A health plan should not become static.

A one-time assessment can tell us a great deal about where you are today. But your health does not stop changing after the test is completed. Your nutrition, sleep, stress, activity, body composition, symptoms, biomarkers, medications, hormones, and goals can all change throughout the year.

Many patients choose membership because they want a clinical team that can continue asking:

  • What is improving?
  • What is not changing?
  • Are the areas we prioritized moving in the intended direction?
  • Has something new become more important?
  • Is the current strategy still justified?
  • Should something be continued, changed, simplified, paused, or investigated further?

“The goal is not to keep adding therapies. The goal is to keep learning from your biology and use that information to make better decisions.”

The Care Loop

Measure. Interpret. Prioritize. Implement. Reassess. Adjust.

01

Measure

Use repeat testing and other objective information to establish where you are and identify meaningful changes.

02

Interpret

An HFW clinician reviews the information alongside your symptoms, history, goals, conventional evaluation, and current plan.

03

Prioritize

Not every abnormal or interesting finding deserves immediate treatment. HFW identifies the areas that matter most now.

04

Implement

Put the plan into practice through appropriate lifestyle changes, nutrition, supplements, medications, hormone or metabolic care, and selected therapies when clinically appropriate.

05

Reassess

Look at symptoms, adherence, biomarkers, body composition, response, and new information.

06

Adjust

Continue what is helping, modify what is not, simplify where possible, investigate new priorities when necessary, and avoid leaving a patient indefinitely on a static plan.

Why TruHealth Is Part of Membership

We use testing to make better decisions, not simply to collect more data.

TruHealth uses DNA methylation-based analysis to provide HFW with a broad advanced view of patterns relevant to health optimization across multiple domains, including metabolic health, nutrients, fatty acids, oxidative stress, mitochondrial function, immune and inflammatory patterns, hormone-related markers, and selected environmental exposures.

  • Metabolic health
  • Nutrients
  • Fatty acids
  • Oxidative stress
  • Mitochondrial function
  • Immune and inflammatory patterns
  • Hormone-related markers
  • Environmental exposures

The value is not simply receiving a report. The value is being able to ask: What should we pay attention to? What can we act on? And when we test again, is the direction of your health changing?

TruHealth adds another layer of information. It does not replace conventional bloodwork, diagnosis, recommended screening, primary care, or other testing when those are needed.

TruHealth DNA test kit and lab samples

A conventional blood panel

Tells us what was circulating on the morning of the draw. It can shift with a poor night’s sleep, a hard training session, hydration, or a single meal.

Methylation-based TruHealth

Reflects patterns your body has been reinforcing over time, rather than a single moment. That makes it a better window into sustained behavior and cumulative exposure, and it is why the same test repeated across the year can show direction rather than noise.

Both matter. TruHealth adds a layer conventional bloodwork cannot see. It does not replace it.

Tracking Change Over Time

One test establishes a baseline. Repeated measurement helps establish a trajectory.

During the first membership year, TruHealth testing is performed approximately quarterly. Each testing cycle creates another opportunity for the HFW team to compare new information with prior results and the patient’s current clinical picture.

The objective is not to make every biomarker “perfect.” It is to determine whether the areas HFW and the patient have chosen to prioritize appear to be moving in the intended direction, and whether the plan still makes sense.

Implementation Support

Knowing what to do and consistently doing it are different problems.

Accountability coaching

Testing and clinical recommendations have limited value if they are never translated into daily behavior. Membership includes health, lifestyle, habit, and accountability coaching designed to help patients implement the plan developed with their clinical team. Coaching may support nutrition, exercise and resistance training, sleep, stress management, habit formation, adherence, and practical barriers to implementation.

The accountability coach does not diagnose, interpret testing, prescribe treatment, or change the clinical plan. Clinical questions and treatment decisions remain with the HFW clinical team.

Patient Success coordination

Members also receive HFW Patient Success coordination for testing, scheduling, reminders, communication, next-step clarity, and routing clinical questions to the appropriate team member. The goal is simple: recommendations should not disappear after an appointment.

Optional Testing

Go deeper when another test can answer a useful clinical question.

Not every patient needs every test. When additional information could change the next step, HFW clinicians may recommend optional testing. Active members receive preferred clinician-review pricing on selected optional tests.

Membership Is Not Required

You can work with HFW without becoming a member.

The HFW Longevity Membership is designed for patients who want ongoing measurement, clinical involvement, implementation support, and reassessment throughout the year. It is not the only way to work with HFW.

Depending on your goals and clinical needs, you may begin with an individual assessment, advanced testing, functional medicine care, hormone care, or a focused therapy protocol without enrolling in membership. During your introductory call, HFW can help determine which starting point makes the most sense.

Membership questions

What people ask before joining

Current pricing and inclusions, stated plainly, including what membership does not cover.

How much does the HFW Longevity Membership cost?

Membership is $225 a month with a 12-month initial term. It begins after a separate initial TruHealth assessment, which is $450 and includes a 60 to 90 minute clinician review of the result. Membership is optional. You do not need to be a member to work with HFW.

What is included in the first membership year?

Four TruHealth tests, approximately quarterly. Four scheduled consultations with an advanced care clinician, plus one additional on-demand consultation. Eight health, lifestyle, accountability, and habit-coaching sessions. Patient Success coordination. Your first quarter of clinician-selected targeted supplements. Ongoing review and adjustment of the plan as each new result comes in. And 15% preferred pricing on eligible peptide and GLP-1 protocol packages, TRT, and HRT.

What is not included in membership?

The initial $450 TruHealth assessment is separate and comes before enrollment. Conventional bloodwork is not included. Medications, peptides, testosterone therapy, hormone therapy, and optional specialty tests are not included either. Membership is not a subscription to a medication, peptide, hormone, or supplement. It is a year-long care system: measurement, clinical interpretation, implementation support, and reassessment.

Do I have to become a member to work with HFW?

No. Depending on your goals and clinical needs you may begin with an individual assessment, advanced testing, functional medicine care, hormone care, or a focused therapy protocol without enrolling in membership. During your introductory call, HFW can help determine which starting point makes the most sense.

What is the initial TruHealth assessment, and why is it separate?

It is $450 and includes the TruHealth test plus a 60 to 90 minute clinician review of your result. It comes first because the membership year is built on comparing new information against a baseline. Without that baseline, the quarterly retests have nothing to be measured against.

How often is testing done during the membership year?

TruHealth testing is performed approximately quarterly during the first membership year, so four tests in total, each followed by clinician review. The objective is not to make every biomarker perfect. It is to determine whether the areas you and your clinician chose to prioritize appear to be moving in the intended direction, and whether the plan still makes sense.

Does TruHealth replace regular bloodwork?

No. TruHealth uses DNA methylation-based analysis to give a broad view of patterns across metabolic health, nutrients, fatty acids, oxidative stress, mitochondrial function, immune and inflammatory patterns, hormone-related markers, and selected environmental exposures. A conventional blood panel reflects what was circulating on the morning of the draw. TruHealth reflects patterns your body has been reinforcing over time. Both matter, and TruHealth does not replace conventional bloodwork, diagnosis, recommended screening, or primary care.

What discount do members get on hormone therapy and protocols?

Active members receive 15% preferred pricing on eligible peptide and GLP-1 protocol packages, TRT, and HRT. Current nonmember hormone pricing is published on the TRT and HRT pages. A listed price does not guarantee that a therapy is appropriate for you or that it is available where you live.

Who will I actually work with as a member?

Your clinical consultations are with an HFW advanced care clinician, under physician medical direction. Your coaching sessions are with an accountability and habit coach, and that role is nonclinical. The coach does not diagnose, interpret testing, prescribe, select therapies, change doses, or manage side effects. Clinical questions go back to your clinician.

Does membership guarantee I will be prescribed a therapy?

No. An advanced care clinician evaluates whether a therapy is appropriate after reviewing your history, risk, goals, and current availability. No prescription or specific therapy is guaranteed by membership. Testing is generally available nationwide. The exact treatment service depends on the patient's state, clinician authorization, laboratory rules, pharmacy requirements, and the therapy involved.

What if I need a test that membership does not include?

Not every patient needs every test. When additional information could change the next step, an HFW clinician may recommend optional testing. Active members receive preferred clinician-review pricing on selected optional tests, including biological age testing, GI-MAP, DUTCH, Metabolomix+, and conventional bloodwork.

Prices shown are current as of Sep 7, 2026. Membership does not include the separate initial TruHealth assessment, conventional bloodwork, medications, peptides, TRT, or HRT. Read about who reviews HFW clinical material, or browse the clinician-reviewed health guides.
